Getting Started

To begin the process of opening an account with Genova Diagnostics at no expense to you, you can download a copy of our Client Information Form and fax it along with a copy of your license that shows the expiration date or a copy of a voided prescription pad form to Customer Support at 828.252.9303. You may also scan the Client Information Form and email it to info@gdx.net.

Once these documents have been received, your account will be opened; a confirmation email sent to you with an electronic copy of our Professional Guide and Fee Schedule. We will also send printed copies of these documents to your office in your Getting Started Packet.
